Short Term Disability

• Coverage is fully employer-paid.
• Short Term Disability provides 100% of base salary for up to 26 weeks.
• Eligibility for coverage is immediate; coverage begins on your date of hire.
  

Long Term Disability

• Long Term Disability (LTD) coverage is based on 60% of your annual earnings to a maximum of $3,000 and 50% of the remainder up to the non-evidence maximum limit of $13,000 (meaning you will not have to provide evidence of health on coverage not exceeding $13,000).
• If your coverage is greater than $13,000, you will be required to provide evidence of health.
• The maximum LTD coverage amount is $15,000, subject to evidence of health.
• The LTD premiums will be deducted from your payroll each pay.
  
• Because you pay the premium for LTD coverage, any LTD benefit paid to you is tax-free.
