- Basic Insurance benefits are employer-provided and include:
- Employee Life: 2x your annual earnings rounded to the next higher thousand to a maximum of $1,000,000
- Employee Accidental Death & Dismemberment (ADD): 2x your annual earnings rounded to the next higher thousand to a maximum of $1,000,000
- Basic Dependent Life – Spouse: $10,000
- Basic Dependent Life – Child: $5,000
- Optional Insurance benefits include:
- Employee Optional Life: up to $500,000 in $10,000 increments
- Spousal Optional Life: up to $500,000 in $10,000 increments
- Spouse Optional ADD: 50% of the employee amount if there are no children or 40% of the employee amount if there are children
- Child Optional ADD: 10% of the employee amount if there is no spouse or 5% of the employee amount if there is a spouse
- Optional Critical Illness benefits include:
- Employee and Spouse Optional Critical Illness: up to $200,000 in $10,000 increments for you and your spouse
- Child Optional Critical Illness: up to $15,000 in $5,000 increments; minimum coverage of $10,000
Pre-existing Condition limitation (applies to Optional coverage that does not require proof of good health)
For amounts issued without proof of good health, no benefits are payable for any covered condition that occurs within 12 months of the effective date of the insured's coverage, and that resulted from any injury, sickness or medical condition whether disagnosed or not for which, during the 12 months prior to the effective date of the insurance, the insured:
- had symptoms;
- consulted a physician or other health care practitioner and was provided any health-related care, advice or treatment; or
- a reasonably prudent person, with such injury, sickness or medical condition or symptom(s) would have consulted a physician or other health care practitioner
