Short-Term Disability Forms and Guides


If you are absent from work for five or more consecutive business days due to your illness or injury you are required to apply for Short Term Disability (STD) per the Canada Sickness and Absence Policy.


Please follow the steps below to initiate a Short-Term Disability (STD) claim:

  • Download the Employee Guide to the Disability Program and follow the instructions.
  • Complete the Employee Statement and Consent form.
  • Have your doctor complete the Physician's Report that corresponds to your condition.
  • Submit the Employee Statement and Physician's Report directly to Canada Life through the secure link located in the guide.
  • Submit a mySolutions Portal Absence Request and enter your leave start date and anticipated return to work date.
  • The request will then be routed to the benefits team. Once benefits team receive the update from Canada life, they will complete the Employer Statement.
  • A Canada Life case manager will be assigned to you and will remain in contact with you throughout the duration of your leave.

The benefits team will remain in contact with Canada Life along with your people leader on the status of your leave and will complete all Workday entries. You do not need to enter any Short-Term Disability time in Workday.
