Short-Term Disability Forms and Guides

If you are absent from work for five or more consecutive business days due to your illness or injury you are required to apply for Short Term Disability (STD) per the Canada Sickness and Absence Policy.

Please follow the steps below to initiate a Short-Term Disability (STD) claim:

Download "Your guide to reporting your absence" and follow the instructions in the guide.
Complete the "Plan Member Statement Salary Continuance Services" form.
Have your doctor complete the "Physician's Statement."
Submit the Plan Member’s Statement (Employee) Statement and Physicians Statement via the mobile app or you can email the documents to disabilityclaims@sunlife.com.
Submit a mySolutions Portal Absence Request and enter your leave start date and anticipated return to work date.
The request will then be routed to the benefits team. Once benefits team receive the update from Sun Life, they will complete the Employer Statement.
A Sun Life case manager will be assigned to you and will remain in contact with you throughout the duration of your leave.

The benefits team will remain in contact with Sun Life along with your people leader on the status of your leave and will complete all Workday entries. You do not need to enter any Short-Term Disability time in Workday.
