Health Benefits

• Annual maximums reset on April 1.
• Co-Insurance (plan pays): Bronze 70%; Silver 80%; Gold 90%.
• Mental Health services are covered up to a combined annual maximum of $2,000 per member.
• Annual paramedical maximums can be utilized in any combination, including a single practitioner.
• Gender affirmation coverage is available to employees enrolled in the medical plan and any eligible dependents (age 18 or older) covered under the plan, up to a lifetime maximum of $25,000.
